periodontal disease treatment

What is periodontal disease

Periodontal disease is a disease around the teeth (gingivitis, periodontitis lesions). It means that the parts around the base of the original teeth are gradually destroyed and absorbed, resulting in a decline in chewing ability. One day, the tooth will be lost, and even good teeth without cavities will face sudden loss. There is no pain in the early stages of the disease. Most people often miss the best opportunity for treatment because they do not understand or ignore the warning signs of periodontal disease.

Why do you get periodontal disease?

The main cause of periodontal disease is poor or incorrect oral hygiene, which allows bacteria (dental plaque, calculus, food residue) to accumulate around the teeth. These bacteria and toxins will first begin to destroy the gums, and then extend to the bones around the teeth (alveolar bone), causing the foundation of the teeth to begin to lose. Other factors such as poor dental restorations or misaligned teeth can also easily cause periodontal disease.

The weapon that destroys periodontal life: dental plaque

Dental calculus is formed by the accumulation of food residues in our mouths and saliva components over time. It will cause the surface of the teeth to become rough, making it easier for bacteria to accumulate. The accumulated bacteria will cause inflammation of the gums, leading to periodontal disease.
In addition to causing gum inflammation, bleeding, periodontal disease, etc., dental calculus sometimes also causes bad breath.

Periodontal disease symptoms?

There are no obvious symptoms of periodontal disease in the early stages. If you notice redness, swelling or bleeding in your gums while brushing your teeth, it is often dismissed as a sign of anger, lack of vitamins, lack of sleep, etc. As the condition worsens, the redness and swelling of the gums will become more obvious, and problems such as gum recession, bleeding, suppuration, bad breath, and tooth shaking will occur. If not treated in time, teeth will eventually need to be extracted. Bleeding when brushing your teeth is basically called gum inflammation. If left untreated for a long time, it will cause alveolar bone to shrink and turn into periodontal disease.

Common symptoms of periodontal disease include the following:
  1. Red, swollen and pus-prone gums
  2. Gums bleed easily when brushing teeth
  3. Bad breath
  4. Gum recession, tooth root sensitivity
  5. There is a dull soreness in the gums
  6. The gaps between teeth are getting bigger and bigger
  7. Increased tooth movement
  8. Teeth shift or become longer
  9. Weakness in chewing

Periodontal disease treatment stages

Phase 1-Basic Treatment

The periodontist uses periodontal instruments deep into the bottom of the gingival sulcus to clean the bacteria (including plaque, calculus, etc.) on the surface and depth of the teeth. In addition, unsuitable fillings or dentures on the teeth need to be reprocessed.
During basic treatment, laser deep sterilization can be used to control inflammation and restore the wound to health. At the same time, oral hygiene maintenance and personal correct tooth cleaning methods will be taught. The need for periodontal surgery will be evaluated after about a month.

Stage 2 - Periodontal Treatment (Surgery/Laser)

Periodontal surgery is a minor surgery, mainly for teeth that still have deep pockets or require surgical treatment after the first stage of treatment.
During the operation, calculus and inflamed soft tissue in deeper areas or root bifurcations can be completely removed. Under appropriate conditions, debridement followed by artificial bone powder and regenerative membrane can create an environment conducive to periodontal tissue regeneration, allowing for better periodontal tissue healing.

Phase 3-Maintenance Treatment

After periodontal disease treatment, regular follow-up examinations and maintenance are required every three to six months. If there is recurrence, it can be detected and treated as early as possible.

Periodontal disease basic treatment program

The basic goal of treatment is to reduce and prevent the accumulation of bacteria on the tooth surface. It includes an initial thorough examination and diagnosis of periodontal conditions, formulation of a preliminary treatment plan, patient education and plaque control, full-mouth teeth cleaning, etc. After completing basic periodontal disease treatment, another periodontal assessment will be performed.
  • Laser Periodontal Treatment
  • periodontal scaling treatment
  • root planing

Periodontal surgery treatment program

The purposes of periodontal surgery include:
(1)Improve the prognosis of teeth with periodontal disease
(2)Correction of bad periodontal tissue morphology to facilitate denture production and aesthetics
Therefore, periodontal surgery can be divided into two categories according to its purpose:

1、Periodontal pocket reduction surgery
  • Periodontal flap surgery: Surgery is performed to open the gums and completely remove the calculus on the tooth root surface.
  • Gingivectomy surgery: Excessive swollen gums caused by genetics or inflammation are removed to create healthy and beautiful gums.
  • Bone modification surgery: The uneven alveolar bone around the teeth is modified to remove the bone defect and reduce the depth of the pocket.
  • Root resection: In small molars or large molars with multiple roots, the root of the tooth with severe periodontal damage is removed to achieve the effect of lowering the pocket and making it easier to clean.
  • Periodontal regeneration surgery: After surgical debridement, biotech materials, regenerative membranes and bone powder are placed to promote the regeneration of periodontal tissue lost due to periodontal destruction.

2、periodontal plastic surgery
  • Crown augmentation surgery: Exposing teeth with insufficient crown length or excessive tooth decay, and trimming the periodontal tissue to increase the length of the crown in order to create beautiful, healthy, and tight-fitting dentures.
  • Keratinized gum grafting: Increases the keratinized gums around the teeth to prevent further gum recession.
  • Root coverage surgery: Cover the exposed tooth roots to improve root sensitivity, tooth decay and aesthetic problems.
  • Aesthetic edentulous augmentation surgery: Use soft and hard tissue to raise the depressions in the gums and alveolar bones caused by tooth extraction, making the dentures that will be produced in the future more lifelike.
Treatment methods will vary depending on the patient's condition. It is recommended that you visit our hospital in person for evaluation and a suitable course of treatment by a professional medical team.
