All-on-4 implant technology is an advanced dental treatment option, especially suitable for patients with severe periodontal disease who are not suitable for traditional implant treatment. In this case, a 50-year-old female patient sought professional treatment and reconstruction advice due to loose anterior dentures. During the initial assessment, we found that her natural tooth roots had been affected by severe periodontal disease, and her gums were red and swollen, so they could not be saved, so we recommended tooth extraction.

The patient's remaining natural teeth in the upper jaw are difficult to preserve, and the alveolar bone is in poor condition. If conventional dental implants are used, multiple surgeries may be required, and the time and cost are difficult to predict. Based on these challenges, we decided to design a treatment plan using the All-on-4 concept, an approach that utilizes a smaller number of artificial roots to provide adequate support at specific locations, thereby achieving a stable and aesthetically pleasing result.

1. Preoperative evaluation

Before starting any treatment, we conduct a comprehensive preoperative evaluation, which includes obtaining complete oral data using 3D digital CT scans and performing detailed analysis and planning of these data. The denture doctor designs the tooth arrangement plan based on the patient's appearance and oral conditions, and asks the patient to try on a model of the future denture to simulate the actual effect and ensure that the design is suitable in all aspects.

2. dental implant surgery

During the dental implant process, we use digital dental implant surgical guides or navigation dental implant equipment to perform the operation accurately. In order to reduce the patient's discomfort, the operation is performed under a soothing sedation state with the assistance of an anesthesiologist.

In this case, we successfully implanted four artificial tooth roots and confirmed that they were stable enough, and then locked them in the All-on-4 denture kit. Before the operation, we have extracted the loose teeth in the upper jaw to restore the gums to a healthy state so that the operation can be performed.

3. Installing temporary dentures

After the operation is completed, the prosthesis specialist will take over, install temporary dentures based on the preoperative simulation results, and make detailed adjustments. The implant placement is completed in the morning, and after a short rest, the dentures are installed in the afternoon. The All-on-4 full-mouth rapid implant procedure can be completed on the same day, allowing patients to quickly restore basic chewing function and oral aesthetics.

4. Installation of formal dentures

After the artificial root has integrated with the alveolar bone, which usually takes about 3 to 6 months, we will install the formal denture and arrange regular checkups to ensure the effectiveness. After communicating with the patient, we finally adopted the All-on-6 design. The palate treatment was divided into two stages:

Phase 1: Four implants are implanted first. If the implants are stable, the upper denture can be worn on the same day to improve the aesthetics and partially restore the bite function. At the same time, sinus augmentation surgery is performed in the left and right posterior teeth areas.

Phase 2: Six months later, another implant is placed at each of the sinus augmentation bone filling sites to form an All-on-6 design. This design extends the dental arch further back, increasing the occlusal area and efficiency, and improving the patient's quality of life.

Through this case, we see the effectiveness of All-on-4 and its extended form All-on-6, not only in solving complex periodontal problems, but also in improving patients’ self-confidence and quality of life. This innovative treatment approach provides a viable and highly effective option for many patients facing similar dilemmas.
